Older adults residing in the community who are 65 years of age and older and: • Have experienced a recent functional decline and have the potential to regain function. • Would benefit from increased supports at home or in the community to remain independent. • Find it difficult to go outside and would benefit from specialized short term geriatric care in the home. • Reside in Service Area: South of St. Clair Ave., West of the Don River, and East of Keele St./Parkside Dr.
