Diabetes Education Programs – Search for a diabetes education program near you and find out how these programs can give your patients the support they need.

Diabetes and You Tool Kit – A unique, engaging tutorial on diabetes, with easy-to-understand fact sheets and short videos.  The kit will help you to further counsel your patients about managing their disease.

Diabetes Prevention Guide   You can provide this booklet or four-panel brochure to your patients.

Diabetes Passport – You and your patient can use the Diabetes Passport and Goal Card to record, track, and monitor important information such as key test results, medications, diabetes education sessions, and more.

Our Child and Teen Diabetes Education Program specialize in infants, children, and teens with Type 1 diabetes, Type 2 diabetes, and those who are at risk of developing diabetes. Our team consists of clerical staff and volunteers, physicians specializing in diabetes and adolescent mental health, diabetes educators including nurses and dietitians, and a social worker. Your child and family are the most important part of the diabetes team. The team will work together to provide care with respect, integrity, excellence, and compassion. Our patients come first in everything we do.

For individuals over 18 years of age living with or at risk of type 2 diabetes.  Programs offer individual counseling and/or group workshops that support people living with or at risk of type 2 diabetes (e.g. those with prediabetes).
