WestReach offers mental health and substance use counseling as well as chronic disease monitoring, weight management, and nutrition education to residents of the Rockcliffe-Smythe and Mount Dennis neighborhoods. Our goal is to minimize the barriers that physicians face when referring clients for mental health and chronic disease services who would otherwise not be able to access these services.

Connections: Weston-Pelham Park Primary Care Interprofessional Team – Serving Weston Pelham Park and Keelesdale-Eglinton.  Our goal is to connect primary care providers in the Weston Pelham Park area with a range of services including specialists and community-based health care services.

TIP is a community-based model of care that addresses complex needs by linking primary care, acute care, and community services.  The TIP nurse works with the patient their family caregiver, and primary care provider to conduct an in-depth comprehensive assessment, which includes determinants of health and patient/family goals.

TIP creates value by:
